Thanks for your comments! I've put an introductory paragraph to add context, as you've suggested here. I'll probably rehaul the survey more completely once I get a few more answers in, and I'll keep your suggestions in mind.

I clicked on the link, saw no explanation of what sort of information would be asked, why it was being asked, what it was going to be used for, any mention of confidentiality at all, and so didn't continue.

So my recommendation is to add some context!
